Overview of your Insperity benefits


          Your Insperity benefits package includes the health and welfare benefits listed in this
          section. Actual benefits are subject to the provisions and limitations of the agreements

          between Insperity and its benefit providers. Detailed benefits information is available online
          at portal.insperity.com.


          The Insperity Group Health Plan



          Medical (insurance carriers vary by region and coverage type)
          Your available medical coverage options are determined by your benefits package and your ZIP code service area. Please see
          “What you need to know about group health plan coverage” for more details. All medical coverage options under the Insperity
          Group Health Plan qualify as minimum essential coverage for purposes of the ACA individual mandate.

          When will my medical ID cards arrive?

          Once you have submitted your enrollment, please allow two weeks for it to be processed by Insperity and your insurer. Although
          your coverage will be effective as of your eligibility date, it can take up to four weeks to receive your ID cards in the mail. During
          this time, you may call Insperity to determine whether your coverage has become active in your insurer’s system. If you need
          health care before you are active in your insurer’s system and/or receive ID cards, you may need to pay out-of-pocket and then
          file a claim for reimbursement.

          What about prescriptions?

          Your prescription coverage is associated with your elected medical coverage option. More information is available in the
          Medical Coverage Options At A Glance section of this guide, as well as at portal.insperity.com.

          Telemedicine options
          Most of Insperity’s medical coverage options include telemedicine (or virtual visit) providers, except where currently prohibited
          by state law. Telemedicine is intended for the treatment of minor illnesses (such as colds, flu, sinus or ear infections), and when
          available with your coverage option, generally costs less than a physician’s office visit or urgent care. Access telemedicine
          providers through your insurer’s member website . Most telemedicine services are available 24/7.

          Wellness programs

          If you elect medical coverage through the Insperity Group Health Plan, various wellness services and discounts such as (but not
          limited to) weight loss and smoking cessation programs or fitness memberships can be found on your selected health insurer’s
          member website (or by calling the toll-free number on your insurance ID card). Additional wellness program discounts may also
          be available through the MarketPlace  on Insperity Premier .

          Dental and Vision
          Although you may elect medical coverage independently of dental and vision coverage, your dental and vision benefits are always
          combined. A summary of each of these benefits is included in the Dental and Vision Benefits At A Glance documents in this guide.

          Insperity’s dental coverage is provided through UnitedHealthcare Dental and member ID cards will be mailed to your home
          address. Insperity’s vision benefits are provided by Vision Service Plan (VSP). No ID card is necessary; simply advise your provider
          of VSP coverage at the time of service.
